What is Support Coordination?

Support Coordination plays a vital role in helping NDIS participants navigate and implement their NDIS plans to achieve their goals. It involves identifying, coordinating and accessing supports and services that meet your specific needs and preferences. This includes:

• Understanding and effectively implementing your NDIS plan to optimise your funding towards your goals
• Connecting you with various NDIS providers, community resources and other support programs
• Assisting you with preparation for NDIS plan reviews.

How Does Support Coordination Help?

Support Coordinators offer choice and control around the types of supports you will need. Once you have secured your NDIS funding, you can begin the process of finding the different supports you will need to meet your goals.

At Interaction, we offer both Level 2 Support Coordination as well as Level 3 Specialist Support Coordination services.

Our Support Coordinators have extensive experience in the NDIS and Disability Sector, allowing us to offer a high level of support to participants with complex requirements. Our person-centred approach allows us to achieve the goals of each child, young person, and adult we support.

Level 2 Coordinators also assist participants in monitoring their NDIS budgets, ensuring the continuity of care needed. Our Level 2 Support Coordinators have experience with Supported Independent Living (SIL) and Specialist Disability Accommodation (SDA) funding, supporting families through the NSW Civil and Administrative Tribunal (NCAT) and obtaining guardianship for their loved one, support with communication with NSW Trustee and Guardian (TAG) and understanding their roles.

Our Level 3 Support Coordinators specialise in addressing complex barriers and collaborate closely with your support team to identify issues and obstacles, assess available resources, develop and implement action plans, and regularly review progress and outcomes to ensure the best quality care is provided. Our Level 3 Support Coordinators have experience and can support participants with forensic supports, Department of Communities and Justice (DCJ) and the Justice system, children under the minister, complex behaviours and restrictive practices, and complex medical needs. Our team of Support Coordinators are experienced in understanding the specific needs of people with any type of disability, including Prader-Willi Syndrome. Interaction also offers support for parents with disability.
